Welcome to the Accessibility Plan Feedback Form

The District encourages feedback on the Accessibility Plan or any accessibility matters within the community.  There are different ways to provide feedback, including:

• Sending an email to accessibility@kitimat.ca,
• Filling out an Accessibility Plan Feedback Form online
• Filling out a paper copy of the Accessibility Plan Feedback Form.  These forms can be picked up from any District facility and returned to Municipal Hall or mailed to 270 City Centre, Kitimat BC, V8C 2H7.

The Freedom of Information and Protection of Privacy Act requires the District of Kitimat to protect the personal information it collects. It also requires the District to use the personal information which it has collected only for the purpose for which it was collected. “Personal information“ includes, but is not limited to, name, civic (home) address, home telephone number, home fax number, personal email address, personal cell phone number and photographs.

The personal information collected on this form is collected for the purpose of assisting in evaluating accessibility matters within the District of Kitimat.
